

5. 30 days after you have been given the name of a
person assigned to your case call that person back and ask if they
have received all the information needed from education and medical
sources to process your application.
6a. If you are not approved ask the Disability
Determination office how you appeal the decision.
6b. If the Disability Determination is approved
they will send the form back to DHS in your community. This is called
a 1645 form.
7. Your DHS income maintenance worker is supposed
to contact Center for Disabilities and Development
and let them know the disability determination is approved.
8. They will mail you a 1644 form that is 12 pages
of questions to determine your child's needs. Complete this form and
return it back to Center for Disabilities and Development. Center for Disabilities and Development will send the 1644
to your DHS worker and inform them to attach it to the 1645 and
forward it to Iowa Foundation for Medical Care to determine the level
of care that will be authorized with State eligibility.
9. After all of this is complete if there is not a
waiting list you will be given authorization for a waiver. If there
is a waiting list you will be informed of what number you are on the
waiting list.
