6b. If the Disability Determination is approved they will send the form back to DHS in your community. This is called a 1645 form.

8. They will mail you a 1644 form that is 12 pages of questions to determine your child's needs. Complete this form and return it back to Center for Disabilities and Development. Center for Disabilities and Development will send the 1644 to your DHS worker and inform them to attach it to the 1645 and forward it to Iowa Foundation for Medical Care to determine the level of care that will be authorized with State eligibility.

9. After all of this is complete if there is not a waiting list you will be given authorization for a waiver. If there is a waiting list you will be informed of what number you are on the waiting list.
