

How to get on a Waiver List
To apply for the Ill and Handicapped, MR or a Home
and Community Living Waiver you must complete the following steps.
The entire process takes about 4 to 6 months, or longer.
1. Call Social Security Office and get a written
denial of eligibility due to income guidelines.
2. Call DHS and ask for a waiver application to be
sent to you. You must identify which specific waiver you want. (NM,
III and Handicapped) Johnson County 339- 6171,

3. After you complete the form and return it to
DHS you will be assigned a worker and be given an appointment to sign
medical/educational releases and review the application.
4a. You may receive an immediate denial because
there is a waiting list for the waiver. You should ask to complete
the disability determination process even though you are on a waiting list,
4b. The income maintenance worker sends the
necessary paper work to Des Moines for Disability Determination
(federal level) 1800-532-1223. You should call them within two weeks
of your DHS appointment to get the name of the person assigned to
your case.
